DCW
RESPONSIBILITIES:
Assisting elderly residents with Activities of Daily Living (ADLs) such as dressing, bathing, transferring, eating, and more. If you are in nursing school and looking for a job to gain experience this could be the job for you!!
requirements:
  1. High School Diploma or GED;
  2. 18 years of age or older;
  3. Ability to Pass Criminal Background Check;
  4. Demonstrated ability to read, write, and effectively communicate;
  5. Strong interpersonal skills;
  6. Driver’s license and reliable transportation;
  7. Certified Nursing Assistant License Optional.
RN
RESPONSIBILITIES:
  1. Follow a designated care plan in accordance with patient's needs;
  2. Make home visits to clients in designated geographic territories;
  3. Perform assigned duties, including administration of medication, wound care, treatments, and procedures;
  4. Monitor clients' conditions; reporting changes to Clinical or Client Services Manager;
  5. Follow up with, execute, and properly document doctors' orders;
  6. Perform client assessments as necessary;
  7. Accurately document observations, interventions, and evaluations pertaining to client care management and services provided, utilizing a state-of-the-art touch pad tablet.
requirements:
  1. A current license as a Registered Nurse in PA;
  2. A minimum of one year of recent, verifiable clinical (medical/surgical) experience;
  3. Prior home care experience strongly preferred, but not required;
  4. Graduation from an accredited and approved nursing program, as indicated by school transcript or diploma;
  5. Demonstrated ability to read, write, and effectively communicate in English;
  6. Ability to work independently and manage time effectively;
  7. Strong interpersonal skills;
  8. Solid computer skills;
  9. Ability to travel to cases as assigned.
LPN
RESPONSIBILITIES:
  1. Assessments;
  2. Training and education of family members;
  3. Medication administration of prescribed treatments and therapies;
  4. Communication with other members of the patients' multidisciplinary team;
  5. Supply management;
  6. Emergency management.
requirements:
  1. Current Licensed Practical Nursing (LPN) license in good standing;
  2. Current CPR certification;
  3. Minimum one-year clinical experience as a Licensed Practical Nurse (LPN);
  4. Strong organizational and communication skills.
OT
RESPONSIBILITIES:
  1. Make home visits to clients in designated geographic territories;
  2. Perform thorough evaluations of patients' functional status, including muscle function, endurance, visual coordination, written and verbal communication skills, and ADL and IADL performance;
  3. Assess cognitive, psychological and motor aspects of performance and develop individualized intervention plans to achieve favorable outcomes;
  4. Evaluate home environment for hazards or barriers to more independent living;
  5. Identify equipment needs and environmental adaptations resulting in enhanced outcomes for clients;
  6. Continually assess and revise the occupational therapy care plan, and participate with nursing in the multidisciplinary care plan, as appropriate;
  7. Educate and instruct patients, family members, or other patient representatives, in rehabilitative care and activities necessary to promote the client's health, safety and independent living;
  8. Accurately document observations, interventions, and evaluations pertaining to patient care management and services provided, utilizing a state-of-the-art touch pad tablet.
requirements:
  1. A current PA Occupational Therapist license;
  2. Prior home care experience preferred, but not required;
  3. A minimum of one year of recent work experience as an Occupational Therapist;
  4. Graduation from an occupational therapy program accredited jointly by the Committee on Allied Health Education and Accreditation of the AMA and the AOTA, as indicated by school transcript or diploma;
  5. Ability to work independently and manage time effectively;
  6. Strong interpersonal skills, organizational skills, and problem-solving skills;
  7. Solid computer skills.
PT
RESPONSIBILITIES:
  1. Document all therapy interventions;
  2. Provide initial and ongoing assessment and evaluation of patient/family response to therapy;
  3. Develop and revise the plan of care in consultation with the physician and other care-team members;
  4. Provide therapeutic treatments and evaluation of equipment needs as applicable;
  5. Advise and consult with the family and other agency personnel as applicable;
  6. Supervise physical therapy assistants at least every 30 days and home health aides every 14 days when appropriate;
  7. Provide discharge planning and evaluation.
requirements:
  1. Successful completion of CAPTE approved PT education program; Current and valid PT license;
  2. Excellent interpersonal communication and patient/family engagement skills;
  3. Flexibility with diverse clients, cultures, and environments allowing you to provide excellent care;
  4. Proficiency with federal, state and governing agency regulations;
  5. Participates in continued clinical and in-service education, as well as professional training required by law;
  6. Graduate of an accredited physical therapy program;
  7. One (1) year experience as a physical therapist (acute care/home care setting) or as required by law;
  8. Current PT licensure (in good standing) in the state(s) of practice.
Pest Control Technician
RESPONSIBILITIES:
  1. Have commercial pesticide applicator certification/license to enable business to apply for a pesticide business license;
  2. Provide expert advice to Management, field technicians and customers on outdoor pest control;
  3. Train and supervise technicians according to federal, state and local regulations;
  4. Provide expert indoor pest control service to customers;
  5. Maintain licensing requirements and proficiency to conduct services;
  6. Ensure company is in compliance with federal, state and local regulations;
requirements:

  2. Maintain a current and valid commercial pesticide applicator certification/license;
  3. Valid driver’s license.
Driver
RESPONSIBILITIES:
  1. Check wheelchair van at beginning of each shift, ensuring vehicle and equipment are ready for the day;
  2. Operate all vehicles in a safe manner adhering to department guidelines and PA vehicle;
  3. Inspect all equipment at start of shift to ensure proper safety;
  4. Adapt to ever-changing transportong needs.

RESPONSIBILITIES:
  1. Maintain record of current job openings and place advertisements as needed;
  2. Review applicants and schedule preliminary phone interviews;
  3. Oversee scheduling process for in-person interviews;
  4. Assist managers in pre-hire procedures including pre-hire appointment scheduling, reference checks, and composing offer letters;
  5. Follow-up with managers as necessary to monitor open position status;
  6. Keep Human Resources Administrator informed of any potential new hires and tentative start dates;
  7. Processing and maintaining complete accuracy of employee information and making certain that all paperwork is up to date and filed properly;
  8. Reviewing employee timesheets and assisting department managers in making any updates necessary to process payroll;
  9. Aiding departments in the employee review process, and sending reminder emails when reviews need to be submitted;
  10. General administrative tasks relating to the daily work of the HR and Payroll department.

LCSW
RESPONSIBILITIES:
  1. Assists the patient, significant others, physician, and health care team staff to understand significant personal, emotional, environmental, and social factors and difficulties related to the patient's health problems; which interfere with maximizing the benefit of medical services and the plan of care.
  2. Contributes as a health care team member to the development of comprehensive, integrated treatment plans for patients;
  3. Instructs health care team members on community resources available to assist patients;
  4. Plans for continuity of care with hospitals and community agencies;
  5. Assesses and treats social and emotional factors related to patient's illness to determine ability to cope with daily living problems;
  6. Assists the patient and significant others to understand, accept, follow, and implement medical recommendations;
  7. Assists the patient and significant others in utilizing community resources which will help the patient to achieve and maintain optimal functioning;
  8. Identifies gaps in community resources and stimulates resource development and/or improvement;
  9. Visits patient according to Plan of Treatment; completes a progress note for each visit; and submits progress notes to the agency on an at least weekly basis;
  10. Participates in staff conferences and committees as necessary;
  11. Provides in-service to agency staff as needed.
requirements:
  1. Master's Degree from a school of Social Work accredited by the Council of Social Work Education;
  2. One year of social work experience in a healthcare setting.
